Assessment of Plant Material Condition
OAC was retained by plant management to provide an assessment of the
plant material condition program. This included its effectiveness and its
overall efficiency. The program was integrated into the plant organization
with the apparent lead being in the maintenance department with involvement
by operations and engineering. Recommendations were provided to enhance
accountability and the overall structure of the program, giving it higher
visibility within the plant management structure to ensure sufficient
technical and operational support were provided when needed.
Site Assessment
OAC was part of a small management team bought from the outside to work
with an internal review group to study the management of a shutdown plant
with the intent of speeding-up the recovery effort. The study reviewed all
aspects of the restart operation including performing a productive-axis
study, allocation and control of technical and management resources, and
establishment of recommendations for management. The outside team provided
leadership and worked with ten members of an internal group during the
study.
Plant Maintenance Project
This project involved reviewing and assessing the maintenance programs
and organization at a production facility. The project considered policies,
procedures, and personnel practices in creating a Maintenance Improvement
Plan. The plan included recommendations for structure, interfaces, and
restructuring policies and procedures to be consistent with appropriate
recommended good practices. A plan was developed and submitted for use by
the contractor work force.
Maintenance Department Assessment
An assessment and diagnostic effort was performed of an operating plant's
maintenance department with the main thrust being establishing methods of
increasing departmental effectiveness. Work flows were investigated from
both the paper process and the physical work process to define where the
structure was not effectively providing work packages to the field staff for
performance. Other issues were discovered that allowed for recommended
restructuring of the crafts and improvements in work control processes. A
productive axis study was performed to define interference in the plant with
inspection and hold points, radiation protection support, and work approvals
by operations.
Special Investigation - Allegations
A special investigation was performed for the President of a company to
assess the validity of allegations of improper and unsafe conduct during a
restart effort after a prolonged shutdown at one of its production
facilities. The primary scope of this project was to review allegations of
unsafe practices and improper actions on the part of management and to
provide an independent assessment. Interviews were conducted with key staff
throughout the organization. Design documents, test procedures, and
modifications to operating procedures were studied to note changes and
potential impacts on safety. Interfaces were also reviewed in depth to
define effectiveness of communications links, allocations of
responsibilities, and conduct of defined responsibilities.
